First Name:		Enter your first name. You must be an existing patient with the Practice. If you wish to become a new patient, call the Doctor's office first. If they are accepting new patients, they will enter you into the system so that you can register here.
Last Name:		Enter your last name. Must match entry at Doctor's office.
User Name:		Enter desired user name. Must be a minimum of 6 characters and contain both a letter and a number.
Email address:		A valid email address is required (note: this email address will not be shared with anyone - see our privacy policy for more information on how we keep your data secure.)
Password:		Enter password to use when you login. Must be a minimum of 6 characters and contain both a letter and a number. Write this password down and keep it secure.
Verify Password:		Type your password again.

Verification Number Type:	SSN Acct # Prim. Ins. Policy #
Verification Number:		Enter either your SSN in format: ###-##-####, or your Acct Number given to you by the office, or your primary insurance policy number on file with the office. One of the three must be used to validate your identity.
Date of Birth:		Enter in the format: MM/DD/YYYY -Used to verify identity
